Incident Reporting and Risk Management
Provides staff with the ability to document and submit critical incident information to allow the customer’s risk manager to investigate, track, and manage the individual incident through to closure. Key information on each incident includes:

- Staff member completing the form
- Date and time incident occurs
- Treatment setting/location
- Severity level
- Status
- Corrective Action
- Client involvement
- Staff Involvement
